With the resurgence of COVID-19 across California, AHHS has turned to telehealth to provide symptom screening, 24/7 monitoring, and virtual visits to high-risk patients in their communities. In the coming months, AHHS will expand their Telehealth program with the aim of reducing costly hospital readmissions.

AHHS will utilize telehealth and RPM to quickly identify potential COVID-19 patients through their recorded symptoms and vital signs. Clinicians can then triage patients through virtual visits, available via the telehealth platform. Telehealth patients will receive a COVID-19 Care Plan with specific educational videos and symptom assessment surveys, as well as a mental health questionnaire to ensure patients are recovering safely and successfully from home. 

Beyond the COVID pandemic, the Telehealth program aims to reduce hospital readmissions and ED visits, helping patients transition from hospital to home. For many patients, round-the-clock care is provided throughout their hospital stay. However, upon discharge, that continuous care is abruptly cut and patients and their families are left to navigate their condition and recovery. Through telehealth, AHHS’ nursing team can support patients and families with continuous monitoring, educational resources, virtual visits and more. 

Patients with low health literacy, chronic conditions, or multiple comorbidities are at the greatest risk for hospital readmissions. As such, AHHS’ Telehealth program is taking a targeted approach to reducing readmissions, enrolling high-risk patients with CHF, COPD, hypertension, and diabetes. In addition, AHHS’ hospice branch, Tranquil Care Hospice is piloting the Telehealth program to expand access to end of life services.

Los fitoesteroles son moléculas similares al colesterol que se encuentran en pequeñas cantidades en todos los alimentos vegetales, incluyendo frutas, vegetales, nueces y legumbres. Los alimentos enriquecidos con altas cantidades de fitoesteroles, principalmente margarina y productos lácteos, se recomiendan comúnmente como parte de una dieta saludable para el corazón, ya que pueden disminuir la absorción del colesterol dietético.

Las proteínas transportadoras NPC1L1 y ABCG5/8 controlan la absorción del colesterol y los fitoesteroles de la dieta. La NPC1L1 transporta los esteroles desde el lumen intestinal a los enterocitos donde la ABCG5/8 excreta menos de la mitad del colesterol pero la mayoría de los fitoesteroles vuelven al lumen intestinal. Así, generalmente absorbemos alrededor del 50-60% del colesterol dietético intestinal pero sólo el 5% de los fitoesteroles dietéticos.

Los autores estudiaron los efectos de las variantes de secuencia que modulan la función del transportador ABCG5/8 en los niveles de colesterol y fitoesteroles en la sangre y el riesgo de enfermedad de las arterias coronarias en grandes conjuntos de muestras de Islandia, Dinamarca y el Biobanco del Reino Unido. Los efectos de las variantes de la secuencia se midieron en hasta 147 mil pacientes con enfermedad de las arterias coronarias y 922 mil individuos sin enfermedad.

El estudio demostró que las personas que albergan variantes de secuencia que disminuyen la función del transportador ABCG5/8 han aumentado los niveles en sangre tanto de colesterol como de fitoesteroles y han aumentado el riesgo de enfermedades cardíacas. Estos resultados confirman que el colesterol alimentario afecta a los niveles de colesterol en la sangre y al riesgo de ataques cardíacos. Los resultados también demuestran que las personas absorben cantidades variables del colesterol que consumen.

El estudio también demostró que el efecto de las variantes de ABCG5/8, que influyen tanto en los niveles de colesterol como en los de fitoesterol, en el riesgo de enfermedades cardíacas, era mayor que el de otras variantes de colesterol que no afectan a los niveles de fitoesterol. Estos resultados apoyan la idea de que los fitoesteroles pueden contribuir directamente a la aterogénesis, lo que plantea interrogantes sobre la seguridad de complementar los alimentos con fitoesteroles.
